Medivac Emergency Ambulance Service Pte Ltd was activated by an insurance provider to perform an urgent cross-border critical care transfer of a 67-year-old Singaporean male from Johor, Malaysia to Singapore following an acute stroke.

Medivac Emergency Ambulance Service received an after-hours call from the insurer requesting immediate evacuation. The patient had suffered a sudden stroke, and according to his son, the hospital in Johor was unable to provide the required neurosurgical or stroke intervention.


Under Singapore Ministry of Health (MOH) guidelines, all critical care transfers—including intubated or high-acuity patients—must receive formal acceptance from a receiving hospital in Singapore before evacuation can proceed.


This posed a significant challenge as the activation occurred over the weekend and after office hours, when most government hospital patient liaison offices were closed, potentially delaying life-saving treatment.

As a MOH-accredited Specified Service Ambulance (SSA) licensed to provide ICU-level interhospital and cross-border transfers, Medivac activated its critical care response protocol.

Our team successfully established direct communication with Tan Tock Seng Hospital (TTSH), a designated stroke centre in Singapore, and obtained formal acceptance for the patient.

An ICU medical team comprising:

  • An Emergency Medicine Specialist
  • An experienced ICU Nurse

was immediately deployed.

Simultaneously, our medical coordination team managed:

  • Hospital-to-hospital clinical liaison
  • Immigration and cross-border clearance
  • Regulatory approvals and urgent evacuation permits

Despite the time-sensitive constraints, the ambulance team reached Hospital Sultanah Aminah, Johor within 3 hours from activation. The patient was stabilized, safely transferred under continuous ICU-level monitoring, and transported directly to Tan Tock Seng Hospital, Singapore, where he received immediate definitive stroke care upon arrival.
